Optional entry points into TouchChart Note module allow nurses or clinical staff to begin patient visit, entering vital signs, patient history and chief complaint in any order. Physicians can then complete the patient visit note by completing the history of present illness (HPI), review of systems (ROS), physical exam, and assessment and plan in their order of preference. After the physician completes the visit and signs off, the E/M code is automatically abstracted from the structured findings documented in the visit note.
